· Requires 400 hours of practice within the past two years
· Contact boardofnursing@state.nm.us to get fingerprint cards and instructions
· NM does state and criminal background check (you pay fees)
· Passport photo (2x2) with your signature on the back of the photo
· Personal checks or cash are not accepted
· Verifies original state of licensure
· Official transcripts may be required if you reside in CA, CT, HI, IL, MI, NV, TN, or WI
· Temporary application requires notarized copy of a current license and a letter from the prospective NM hospital on their letterhead
