· Include a copy of a current license if you apply for a temporary license
· Personal checks not accepted
· Require original passport style photo
· Background check is required and must be notarized
· Fingerprint cards are required and MUST come from FBI. Call 501-686-2710 to have cards mailed to you
· Fingerprint cards and background check must be mailed to Arkansas State Police with two separate checks: $15 and $24. Address is included on form.
· Transcripts required if original license from Florida or Utah
